  1. Accomplished Unified Application Form
  2. DTI Certificate for Sole Proprietorship; or SEC Certificate for Corporation; or CDA Certificate for Cooperative
  3. Barangay Business Clearance
  4. Photo or sketch of business location.
  5. Real Property Tax Clearance; or Stall Rental Clearance
  6. Occupancy Permit (if business is renting in a commercial building)
  7. Contract of Lease (if lessee)
  8. Health Card/s (if food handler)
  9. Other selected sectors with required permits/Clearances from other Government Agencies. Please see ANNEX 5.

Requirements (RENEWAL):

  1. Accomplished Unified Application Form
  2. Statement of Gross Sales of previous year (notarized)

For verification of expired or lapsed validity period:

  1. DTI Certificate for Sole Proprietorship; or SEC Certificate for Corporation; or CDA Certificate for Cooperative
  2. Barangay Business Clearance of previous year.
  3. Photo or sketch of business location
  4. Real Property Tax Clearance; or Stall Rental Clearance
  5. Contract of Lease (if lessee)

Requirements (RETIREMENT OF BUSINESS):

  1. Accomplished Application for Retirement Form
  2. Original Copy of the Recent Business Permit
  3. Letter of Intent indicating reason for closure of business (notarized) – see sample
  4. Statement of Gross Sales from January to December of the previous year (notarized)
  5. Board Resolution or Certificate of Dissolution (for Corporation) – see sample
